Home Health Care Through Northwoods Home Health and Hospice

Home health care is available to anyone in need of everyday assistance after an acute illness, injury or exacerbation of a chronic illness. Home health care providers help patients remain in the comfort of their own home rather than a more costly healthcare facility.  Services are provided through Weeks Medical Center's Northwoods Home Health and Hospice.

Home Health is a Medicare-certified agency offering the following:

The Home Health staff is skilled in providing:

  • Wound care
  • Home IV therapy
  • Newborn and infant care
  • Cardiac care and Congestive Heart Failure management
  • Medication management and instruction
  • Post surgical care including joint replacement therapy
  • Post cardiac surgical monitoring and instruction
  • Personal care assistance
  • Home audits to insure patient safety
  • Pain Management

Home Health care is paid for by Medicare, Medicaid and most third party commercial insurance carriers.

Hospice Care

Hospice is a centuries-old concept rooted in the idea of providing a place of shelter and rest to sick and weary travelers on a long journey. Hospice care provides humane and compassionate care for people in the last phases of incurable disease so that they may live as fully and comfortably as possible.

The hospice philosophy recognizes death as the final stage of life and seeks to enable patients to continue an alert, pain-free life and to manage other symptoms so that their last days may be spent with dignity and quality, surrounded by their loved ones.

The Hospice staff provides pain and symptom management, assistance with personal care, emotional support, and teaching care of the terminally ill, all with the goal of providing quality end-of-life care.

Hospice is a Medicare-certified agency providing:

Medicare and most third party commercial insurers pay for Hospice services.

Hours of operation: Monday – Friday, 7:30 am to 4:00 pm

On call coverage: Monday – Friday, 4:00 pm to 7:30 am and all day Saturday, Sunday and holidays.
